1. Are you superstitious?
Yes. More than anyone else in my family, and most of my friends.
2. What extremes have you heard of someone going to in the name of superstition?
My grandma made my dad turn the car around and go a different way (to church, of all places) after a black cat ran across the street in front of them.
3. Believer or not, what's your favorite superstition?
I don't think I've ever thought of them as favorite/least favorite. They just are. But one that makes me happy and reminds me of my grandma is when you say "bread and butter" if you let an item (telephone pole, street sign, etc.) pass between you and a friend as you are walking together. For some reason, saying "bread and butter" immediately after walking on either side of such an obstacle will keep things from coming between you and your friend.
4. Do you believe in luck? If yes, do you have a lucky number/article of clothing/ritual?
I definitely believe in luck, but I don't think it's brought about or kept by tangible items. I think some people are lucky and others aren't. My dad is one of the luckiest people I know, but he's the least superstitious person in the world. I think people make their own luck. If you honestly believe good things will happen to you, it's more likely that those good things will happen. It's the truest form of magic.
5. Do you believe in astrology? Why or why not?
By "believing in" I assume the question means "do you follow it?" Which I have to answer, not really. I think astrology is similar to tarot cards. Both give you an outside view to a problem and you may interpret that view in multiple ways. Both can be used as meditative tools, but I don't think they give definitive answers to problems.
